New York IVF Insurance Mandate (A7549)
Large group health insurance plans (100+ employees) regulated by New York must cover 3 IVF cycles. Small group and individual plans must cover infertility diagnosis and treatment but not IVF.

Ask HR: (1) Is our health plan state-regulated or self-insured under ERISA? (2) If state-regulated and 100+ employees, NY law requires 3 IVF cycles. Self-insured (ERISA) plans are NOT subject to the mandate. (3) Get the answer in writing -- many HR reps don't know.
